Student and Family Handbook + Agreement Form

Student and Family Handbook CoverDownload a copy of the current UChicago Charter School Student and Family Handbook as well as the school policy agreement form below. All current families should receive a hard copy of the current handbook from their campuses. The handbook contains all policies and procedures for the school, including attendance, school fees, discipline, and technology use policies. The most updated version will be uploaded here as it comes available:

Download a copy of the student media release form — all students and families sign this form upon enrollment.

Online Services Consent Form (COPPA)

Federal law, specifically the Children's Online Privacy Protection Act (COPPA), requires that children under 13 years of age must have verifiable parental consent to use online services that collect personal information. COPPA also permits a school to gather consent from parents on behalf of its students, thus eliminating the need for parents to provide consent and establish accounts individually with each of the service providers.

Please print and sign a copy of the Online Servicess Consent Form and return to the main office of your school. To simplify our record keeping, we ask that you renew this document each year until your student reaches the 9th grade.
